Az Business Magazine is seeking nominations for the 8th Annual Health Care Leadership Awards.
We want to hear from you! If you know of someone making a difference in Arizona’s health care community or helping Arizonans receive better care, please tell us their story! AZBusiness is looking for physicians, specialty care providers, surgeons, nonprofit leaders and volunteers, hospital administrators and CEOs, dental providers and health care managers and nurses.
Deserving candidates may be nominated in the following categories:
- Community Outreach
- Dentist
- Hospital Administrator
- Hospital Executive
- Institution or Educational Program
- Insurance Provider
- Manager
- Medical Center or Hospital
- Nurse or Nursing Advocate
- Physician
- Researcher
- Surgeon
- Volunteer (unpaid)

Nomination forms must be completed and sent in by Monday, December 29th, 2014 to be considered.
ALL sections of the form must be completed for consideration by the selection committee.

All nominations will be reviewed by a selection committee comprised of industry leaders and the Arizona Business Magazine editorial team. Selection committee will determine all finalists and winners.
